3. On the first day of the month following ratification of this memorandum,
amend benefits aa followa:

Extended Jealth Benefits

Increase eyeglasses maximum to $250.00

Group Lite

Increase maximum Group Life benefit to $100,000 and adjust AD &
D accordingly

Dental

Plan "A" - increase benefit to 90%
Plan "C" ~ add adults coverage with existing limits

4, Amend contract language as follows:
