RENEWAL RIDER

to be attached to and form a part of the Dental Care Plan Service Contract,
D115, issued by the Medical Services Association, Vancouver, B.C., to
THE CORPORATION OF THE CITY OF PORT COQUITLAM.

Subject to all the terms, provisions and conditions thereof, effective
January 1, 1976, said Contract is hereby continued in force for a

further term of one year ending December 31, 1976 at 11:59 P.M., at the Em-
ployer's address at the following rates:

PLAN "A" PLAN "B"
Employee $ 4.30 $2.24
Employee and dependents $12.04 $ 4.46

This Rider shall not vary, alter or extend the terms, provisions or con-
ditions of said Contract except as herein expressly set forth.
