the following information: :

1.. Size of Lot and Buildings

2. Siting of all buildings to include measurements of setbacks from
property lines

3. Siting of proposed addition to be shown in dotted lines and to show
setbacks from property lines :

4. Site plan to be shown below or on 4 separate sheet no larger than
Bhs" x 14"
